Insurance and Billing
Dr. Jarosz is considered an “out-of network” provider for all insurance plans and non-preferred Medicare provider. This means payment in full is due at the time of the visit. Please note we do not contract with any insurance companies. We will be happy to provide you with a superbill which will include all of the information your insurance will need for potential reimbursement. You can submit it to your insurance company for reimbursement, generally as an “out-of-network provider.” However, the superbill does not guarantee that an insurance provider will in fact reimburse you. Insurance coverage for osteopathic treatment varies between plans and carriers. Please check with your plan support team regarding coverage details. Prolotherapy and PRP treatment are not covered by any insurance, including Medicare.

What to wear
It is best not to wear jeans, skirts, dresses or other tight clothing. Please plan to empty all pockets and remove belts, jewelry and shoes prior to treatment. Please dress comfortably in flexible/cozy and loose clothes.
Appointment Time
Please arrive 10-15 minutes prior to your appointment. It would be best to submit all completed forms online prior to your appointment start time. Patient registration will be sent to you via our medical records software.
What to expect at your first OMM consult?
Your initial appointment will include a detailed review of your lifestyle, diet, medical and exercise history, medications, emotional wellbeing and thorough physical examination, followed by a hands-on exam and treatment plan synthesized to optimize self-regulating, self-recovery and self-healing.

What to expect after your OMM treatment?
Treatments may lead to powerful physiologic and structural changes within your body. These changes may affect you differently each time. You may feel anything from immediate relief to a temporary increase in soreness. Those adjusted symptoms may resolve within 24 to 48 hours. Please drink adequate amount of water (room temperature).
It would be best to avoid other treatments such as physical therapy, acupuncture or vigorous activity for the first 24 hours post treatment to allow for healing process to continue without interference. Resuming your “regular” level of activity is recommended, however take it easy for the first 24 hours post-treatment. Please defer fitness training and other body-work appointments until after 24 hours post-treatment.
